We are honored by your interest in our 4th annual Awaken Your Divine Heart Expo! Please note that all Vendor
applications are subject to approval by the Awaken Your Divine Heart Expo Committee. After approval, you will
be contacted regarding payment of Vendor Rental space, including optional electricity*. Our vision is to provide
a successful and enlightened Holistic Expo!
VENDOR RENTAL SPACE DESCRIPTION:
• Vendor Rental Space is $100.00 - Size is approximately 10 ft. by 10 ft. (concrete flooring)
• One 8 ft. table included (you will need your own table covering)
• Two metal folding chairs included
• Electricity optional $25.00 (one outlet - bring your own extension cord, surge protector, etc.)
*The Vendor spaces with electric are limited and go quickly, so let us know ASAP if you need electric.
We require that Vendor displays be neat, in good taste, products and services properly labeled, free of fire
hazards (no burning flames, incense, etc.), and that noise be kept to a minimum to respect other Vendors and
Expo Attendees. All taxes, licenses, and insurance are the sole responsibility of you, the Vendor.
SET UP INFORMATION:
Registration and Set-up on Saturday, November 12, 2011, from 8 to 9:30 a.m. in the Youth Hall at the Richland
County Fairgrounds, 750 W. Home Road, Mansfield, Ohio. (You will see signs directing you to the Youth Hall,
and will be directed where to park after unloading.)
Please register upon arrival at the Admission/Registration Table where you will receive a name tag and will be
directed to your specific Vendor space. Door Prize information will also be exchanged at this time (see below).
Unloading and clearing all aisle-ways to be completed no later than 9:30 a.m. (If more set-up time is needed,
please contact Diane Leicy 419-571-0538.) The opening Awaken Drum Ceremony begins at 9:45 a.m., before the
Expo open at 10 a.m.
NEW THIS YEAR: Vendor rental spaces need to remain open throughout the show until 5:00 p.m., at which time
you are invited to participate in our new Divine Drum Circle! Please keep at least one person at your booth at all
times (up to 5 p.m.), for security/safety purposes.
DOOR PRIZE INFORMATION:
We ask that each vendor contribute a Door Prize that will encourage Expo attendees to stop by to see you! Upon
registering, you will initial a Door Prize Card with your Vendor Name and on it, and by doing so agree to offer
your free prize to the winner presenting this card to you at your Vendor rental space. Winners will be drawn
every half-hour from tickets obtained at the door upon entrance to the Expo. The winner’s names will be
displayed at the Admission Table and may also be announced throughout the day.

Vendor Rental Contract

TERMS AND CONDITIONS:
This Contract is made between the Awaken Your Divine Heart Exposition Committee (hereinafter called
AYDHEC), and you the vendor (hereinafter called Vendor). AYDHEC, in consideration of the rent herein
contained, rents to Vendor, and Vendor lets from AYDHEC, the following described premises, (herein after
called Premises) being a 10’ x 10’ Vendor Rental Space, at the Youth Hall Building at the Richland County
Fairgrounds, located specifically at 750 N. Home Road, Mansfield, Ohio.
This Contract is entered upon the following terms and conditions:
1. The term of the Contract shall be specifically for the 12th day of November, 2011, and continue throughout
the duration of this Event. Vendor’s failure to not maintain Vendor Rental space display during this
aforementioned time will not be invited to return the following year. For security and safety purposes, the
Vendors Rental space will not be left vacant at any time period, until up to 5 p.m. during the open Vendor hours
of the Expo.
2. Vendor shall pay the Awaken Your Divine Heart Expo Coordinator, Diane Leicy, at 2974 Mabee Road,
Mansfield, OH 44903, rent in the sum of $100.00 per 10’ x 10’ Vendor Rental space, due by the 15th day of
October, 2011. The optional $25.00 fee for electric is also to be paid in full no later than October 15, 2011. If total
Vendor Rental space and optional electric payment is received no later than July 31, 2011, you will receive a $25
discount! Payments received after October 15, 2011, will be charged $25 late fee and are a default under this
contract which could result in forfeiture of Vendor Rental space and electric, if applicable. Checks and money
orders will be accepted either in person or by mailing to AYDHEC Coordinator: Diane Leicy, 2974 Mabee Road,
Ontario, Ohio 44903-9116, no later than the 15th day of October, 2011. To make payment arrangements, call

3. Compliance with any and all requirements for permits, licenses, tax payments required in the conduct of
business in this show, plus securing insurance coverage, is the sole responsibility of the Vendor.
4. Vendor shall not use, or permit the use of, Premises for any unlawful purpose or in violation of any law,
order, or regulation of any governmental authority or any restrictive covenant relating to the use or occupancy of
the Premises. Vendor shall not permit any waste or misuse of the Premises. AYDHEC reserves the right to
disconnect any equipment which deems to be a fire hazard. Vendor shall not display any object deemed offensive
or in poor taste, nor any material that would constitute a fire hazard, at which time Vendor agrees to remove, at
the request of AYDHEC, any object that is offensive, in poor taste, or potential fire hazard.
5. Vendor will correctly represent and label all products and/or services exhibited or offered for sale on the
Premises. It is the Vendors obligation to comply with any and all governmental requirements for conducting its
business on the Premises in the manner proposed. Vendor will keep noise level to a minimum out of respect for
other vendors and booth attendees. Vendor agrees to be respectful and cooperative with other vendors, the
AYDHEC and attendees. AYDHEC reserves the right to settle all disputes between Vendors.
6. Vendor takes full responsibility for loss arising from damage and/or loss of Vendors personal property
and trade items located on the Premises. Vendor saves AYDHEC and its members, harmless from loss, cost or
expense by reason of claims for personal injury and property damage arising out of Vendor’s occupancy of the
Premises, whether due to the fault of the Vendor or others. In the event that AYDHEC is unable to hold the Expo
at the location and time specified above due to any cause outside of AYDHEC’s control, Vendor waives any
claims for damages except that the Vendor will be entitled to a refund of any monies actually paid to the
AYDHEC Coordinator for the Awaken Your Divine Heart Expo.
7. Vendor will not sublet the Premises or any part thereof be used by others. If vendors would like to share a
Vendor Rental space, there will be a limit of two (2) Vendors per 10’ x 10’ booth space, and is subject to approval
by the AYDHEC. Each vendor will need to complete and submit a separate Vendor Application form and signed
Vendor Contract, noting the shared booth when submitting.
8. Electric is available for an additional $25.00 fee, payable at the same time as the Vendor Rental space fee as
stated above, by the 15th day of October, 2011. Each Vendor will provide their own extension cord, surge
protector and anything else needed to operate equipment within said Vendor Rental space. Please note that there
will be only one electrical outlet available per Vendor who has additionally paid for electric.
9. Vendor will not make any alterations and/or additions to the Premises, including and without limitation to
painting, drilling or attaching fixtures. Violation will result in termination of this Contract and Vendor shall be
liable for any damage to the Premises caused by Vendor or Vendor Agents in removing and/or destroying
property on the Premises.
10. This contract can be cancelled by either party, in writing only, up to 12 days before event, at which time
the previously paid booth fee and optional electricity, will be refunded to the Vendor, minus a $25.00 handling
fee. If the Vendor does not provide 12-day written notice, except in cases of acts of nature beyond the control of
the Vendor, it is agreed that no monies will be refunded.
11. Any changes in this contract must be stated in writing and be approved by the AYDHEC Coordinator and
Vendor.
CONTRACT ACKNOWLEDGEMENT
I acknowledge that I have read both the Vendor Information and the Vendor Rental Contract and agree to abide
by the terms and conditions of this Contract. I agree to be responsible for all Vendor Rental Fees, including
optional electric, to be paid no later than October 15, 2011.
